Our Clinical Mental Health Community Support Service

This includes Individual outreach and clinic-based support at Mopane CMHS premises. We will work in partnership with you, your family, your medical practitioners and other allied health to reduce the impact of your mental illness, improve your quality of life and to promote recovery.

We aim to be accessible to all and are sensitive to the diversity of our community. We cover the local government areas of: Northern Suburbs, Darebin, Whittlesea, Moreland, North and West Metropolitan areas.

This service is provided by highly experienced and qualified Credentialed Mental Health Nurses alongside Accredited Mental Health Social Workers. For our clients we can arrange and facilitate Psychiatrists appointments without out of pocket payments/expenses.

Program Services

  • Targeted Psychological Services (TPS), Suicide Prevention Services: SPS (Funded via local Primary Health Network – PHN)
  • Assess skills, ability, needs of eligibility for relevant Funding like NDIS and coordinate care support.
  • Referral of eligible clients to local Mental Health Community Support Service providers of choice for comprehensive assessment, Physical health and well being.
  • Follow up of referral to service providers to ensure timely access for the client or participant through case coordination, Medication management and administration
  • Transcultural Psychiatry, Mental Health and spirituality
  • Provide these services for individuals with NDIS funding, Individual Recovery Plan
    Facilitation of finding appropriate accommodation for youth/adult residential rehabilitation services.
  • Referrals to Carers and family support services, Advocacy
  • Behaviour management and stress and resilience strategies

Regulatory Bodies

We have robust outcome measures and our clinicians are regulated by regulatory bodies such as the Australian Health Practitioner Regulation Agency (APHRA), Australian College of Mental Health Nurses and the Australian Association of Social Workers (AASW).

We offer crisis response strategies, but we are NOT a crisis response team like the Public funded Crisis Assessment and Treatment Team CATT. For emergency services please contact/call:

Who Can Access the Program

This program supports people aged 16-64 that have a disability that is attributable to a severe and enduring psychiatric condition. Also those who have impairment/s that are permanent or likely to be and that results in substantially reduced psychosocial functioning that affects communication, social interaction, learning, and self-care, self-management social and economic participation.

Eligibility and funding arrangements such as NDIS, Targeted Psychological Services (TPS) and, Suicide Prevention Services (SPS) can be accessed. Private funding accepted and a management support plan can be negotiated.
